More than 3,400 Koreans earn nearly W1 billion a year (US$1=W1,162).
According to the National Health Insurance Service on Monday, 3,403 Koreans paid the maximum W2.39 million a month in health insurance fees since their monthly salary exceeded W78.1 million.
An NHIS staffer said most are CEOs of major conglomerates, lawyers and doctors.
The number of Koreans paying the maximum monthly health insurance fees has risen fast, from just 2,508 in 2012.
The ministry sets 30 times the average monthly wage of W2.6 million as the maximum limit for health insurance fee calculation and charges W2.39 million beyond that point.
